Plasma volume, the fluid carrying the blood cells, can be estimated from weight and hematocrit with the Kaplan formula: one minus hematocrit times a sex-specific constant plus a weight term. Comparing the estimate with ideal plasma volume gives plasma volume status, the percent deviation from ideal, which Duarte and colleagues validated as a congestion marker in heart failure: a status above minus 4 percent signals volume overload. This calculator runs all three numbers for clinical teaching and reference.

How to Use This Calculator

Enter sex, weight, and hematocrit from a recent CBC. The calculator estimates actual plasma volume, ideal volume, and volume status.

1

Use a recent hematocrit and current weight. Both shift with fluid status, so same-day values matter.

2

Pick the correct sex: the formula constants differ because body composition differs.

3

Read plasma volume status. Duarte and colleagues showed values above minus 4 percent track congestion in heart failure.

How the Math Works

Actual PV = (1 - Hct) x (a + b x weight kg); men a=1530 b=41, women a=864 b=47.9 Ideal PV = 39 (men) or 40 (women) mL/kg. PVS = (actual - ideal)/ideal x 100

Actual plasma volume in mL equals one minus hematocrit times the quantity a plus b times weight in kg, with a 1530 and b 41.0 for men, a 864 and b 47.9 for women. A 70 kg man with hematocrit 45 percent: 0.55 times 1530 plus 2870, which is 0.55 times 4400, equals 2420 mL. Ideal plasma volume is 39 mL per kg for men and 40 for women, so 2730 mL here. Plasma volume status is actual minus ideal over ideal times 100: minus 11.4 percent, indicating no congestion signal. The hematocrit term corrects total blood volume down to its plasma fraction.

Worked Example

A 70 kg man with hematocrit 45 percent has an estimated plasma volume of 2,420 mL, against an ideal of 2,420 mL. The plasma volume status is 2,730 mL, which the calculator reads as -11.4%%: comfortably below the minus 4 percent congestion threshold. The same man retaining fluid would show falling hematocrit and rising status, which is exactly the pattern heart failure teams watch.

Reading Your Result

Plasma volume status at or below minus 4 percent suggests euvolemia to contraction; above minus 4 percent suggests congestion and predicts worse outcomes in heart failure cohorts. Use it as a trend: rising status across visits means expanding plasma volume before weight or exam catches it. It complements natriuretic peptides and exam, never replacing them.

Limitations

The formula estimates from population constants; individual body composition, anemia causes, and acute shifts distort it. It assumes stable hematocrit, so active bleeding or transfusion breaks the math. PVS is validated in heart failure populations and generalizes cautiously beyond them. It informs volume assessment; the exam and clinical picture decide diuresis.

Frequently Asked Questions

What is plasma volume?

The fluid portion of blood, about 55 percent of blood volume, carrying cells, proteins, and solutes. Roughly 2.5 to 3 liters in adults. It expands in fluid overload and contracts in dehydration and bleeding.

How is plasma volume estimated?

With the Kaplan formula from hematocrit and weight, no tracer needed. Gold-standard measurement uses radiolabeled albumin dilution, which is accurate but impractical for routine care. The formula trades precision for availability.

What is plasma volume status?

Actual estimated plasma volume minus ideal, divided by ideal, as a percent. Duarte and colleagues showed it predicts congestion and outcomes in heart failure better than the raw volume. Above minus 4 percent flags overload.

Why does hematocrit appear in the formula?

Because plasma is what remains after subtracting the cell fraction: one minus hematocrit is the plasma fraction of blood. Hemodilution from fluid retention lowers hematocrit and raises estimated plasma volume, which is the signal.

What is ideal plasma volume?

39 mL per kg for men and 40 for women in the Hakim formulation. It is the reference point the status calculation measures deviation against.

How does this help heart failure care?

By quantifying congestion objectively: rising plasma volume status can precede weight gain and symptoms, guiding diuretic decisions. Studies link elevated status to hospitalization and mortality risk.

Does anemia distort the estimate?

Yes. Anemia from any cause lowers hematocrit and inflates estimated plasma volume without true expansion. Interpret the status alongside hemoglobin trend and cause of anemia.

Can athletes use this?

Endurance training expands plasma volume substantially, so athletic baselines differ from population formulas. The tool is built for clinical volume assessment, not sports physiology.

Plasma volume vs blood volume?

Blood volume is plasma plus cells; plasma volume is the fluid part. Transfusion and bleeding change both, while pure fluid shifts change plasma mostly. The hematocrit term converts between them.

When should this be measured directly?

Rarely: labeled-albumin dilution is reserved for research and puzzling cases like refractory edema or unexplained anemia. The estimate serves everyday clinical reasoning.
