Estimated VLDL cholesterol
For informational purposes only, not medical advice.
VLDL cholesterol is estimated as triglycerides divided by 5, in mg/dL. Triglycerides of 180 give a VLDL of 36 mg/dL. The ratio works because VLDL particles carry triglycerides and cholesterol in a roughly 5-to-1 mass ratio, a relationship the Friedewald equation has exploited since 1972 to complete the lipid panel from three measured numbers.
Most lab reports never print VLDL; they print triglycerides and let the equation do the rest behind the scenes. Computing it yourself demystifies where the LDL number on your report actually comes from.
How to Use This Calculator
Enter fasting triglycerides in mg/dL or mmol/L. The tool converts units, divides by five, and flags when triglycerides are too high for the estimate to hold.
Enter triglycerides from a fasting lipid panel and select the unit.
Read the estimated VLDL in mg/dL.
Note the reliability flag: above 400 mg/dL triglycerides, the 5-to-1 ratio breaks down and direct LDL measurement beats calculation.
The Friedewald Relationship
VLDL is the liver's triglyceride delivery truck, and its cargo ratio is remarkably constant in most people: five parts triglyceride to one part cholesterol. Friedewald's insight was that total cholesterol minus HDL minus this estimated VLDL leaves LDL, the number guidelines treat. The whole standard lipid panel is therefore two measurements and one division away from complete.
The ratio fails when triglycerides soar past 400, in chylomicronemia, and in type III hyperlipoproteinemia, where abnormal particles carry different cargo. Modern labs increasingly measure LDL directly or use the Martin-Hopkins equation with an adjustable ratio; the divide-by-5 remains the classic every clinician should know.
Worked Example
Triglycerides 180 mg/dL: VLDL is 180 divided by 5, or 36 mg/dL. If total cholesterol is 220 and HDL is 50, then LDL is 220 minus 50 minus 36, which is 134 mg/dL, the borderline-high band.
That chain shows why triglycerides move LDL on paper: at triglycerides of 300, VLDL becomes 60 and the same total and HDL give an LDL of 110 instead of 134. Treating triglycerides lowers the calculated LDL even though the LDL particles themselves did not change, a quirk worth knowing when watching numbers move.
Reading Your Result
| VLDL | Context |
|---|---|
| Under 30 mg/dL | Typical range corresponding to triglycerides under 150. |
| 30 to 80 mg/dL | Elevated; tracks with higher triglycerides and remnant risk. |
| Over 80 mg/dL | High; triglycerides above 400 make the estimate itself unreliable. |
My take: VLDL is a supporting actor, but remnant cholesterol (total minus HDL minus LDL) is the emerging star for risk prediction. If your report shows high triglycerides with normal LDL, ask about remnant risk rather than celebrating the LDL alone.
Limits
Divide-by-5 assumes fasting samples and normal particle composition; non-fasting triglycerides, severe hypertriglyceridemia, and dysbetalipoproteinemia all break it. Unit errors are the classic pitfall: in mmol/L the divisor is 2.2, not 5, which is why the tool converts for you. And VLDL alone never sets treatment; the full lipid and risk picture does.

Frequently Asked Questions
What is VLDL cholesterol?
Very-low-density lipoprotein cholesterol: the cholesterol carried in triglyceride-rich particles secreted by the liver. It is estimated as triglycerides divided by 5 in mg/dL and forms part of the Friedewald LDL calculation.
How do you calculate VLDL?
Divide fasting triglycerides by 5 when using mg/dL, or by 2.2 when using mmol/L. Triglycerides of 180 mg/dL give VLDL of 36 mg/dL. The estimate is unreliable when triglycerides exceed 400 mg/dL.
What is a normal VLDL level?
Typically under 30 mg/dL, corresponding to triglycerides under 150. Higher values track with elevated triglycerides and increasing remnant lipoprotein risk.
Why divide triglycerides by 5?
VLDL particles carry triglycerides and cholesterol in roughly a 5-to-1 mass ratio in most people. The Friedewald equation uses this fixed ratio to estimate VLDL from measured triglycerides.
What is the Friedewald equation?
LDL = total cholesterol - HDL - (triglycerides/5), all in mg/dL. Published in 1972, it made LDL calculable from a standard lipid panel without ultracentrifugation. It fails when triglycerides exceed 400 mg/dL.
Is VLDL bad cholesterol?
VLDL and its remnants contribute to atherosclerosis, though LDL remains the primary treatment target. High triglycerides with high VLDL signal remnant risk that LDL alone misses.
Should triglycerides be fasting for this?
The classic equation assumes fasting samples, since meals spike triglycerides and distort the ratio. Many modern labs accept non-fasting panels but interpret them with adjusted thresholds.
What if triglycerides are over 400?
The divide-by-5 estimate becomes unreliable. Labs then measure LDL directly or use equations with adjustable ratios like Martin-Hopkins. Very high triglycerides also raise pancreatitis risk, a separate concern.
How can I lower VLDL?
The same steps that lower triglycerides: less refined carbohydrate and alcohol, weight loss, regular exercise, and treating diabetes. Medications like fibrates or prescription omega-3s target severe elevations.
Who should interpret my lipid panel?
Your clinician, in the context of overall cardiovascular risk, not any single number. This calculator teaches the VLDL arithmetic; treatment decisions use the full panel plus risk factors.
